Student responsibilities:

  1. Students contact the DRC to determine their eligibility for accommodations. Students will request and receive their accommodation letter. This letter is provided to the instructor. This is official notice of a request for accommodation.
  2. Students then notify their instructor during office hours or after class of their accommodations, and provide their instructor with their Accommodation Letters.

  3. Students will manage their own disclosure of disability status. Please maintain this confidential information according to UC Santa Cruz data practices.

Instructor responsibilities are as follow:  

1.  Instructor makes an announcement during the first two weeks of class, asking students to come to them after class or during office hours to discuss testing accommodations. Additionally, the instructor should make an announcement a week before each exam, for any newly approved DRC students. It is also recommended that instructors post the following DRC policy information on their syllabi or course webpage:

UC Santa Cruz is committed to creating an academic environment that supports its diverse student body. If you are a student with a disability who requires accommodations to achieve equal access in this course, please submit your Accommodation Authorization Letter from the Disability Resource Center (DRC) to me privately during my office hours or by appointment, preferably within the first two weeks of the quarter. At this time, I would also like us to discuss ways we can ensure your full participation in the course. 2.  Instructor submits all testing dates as soon as possible to ISS via Google Web Form found here.

Note: Instructors should submit  accommodation requests to ISS no later than 7 days before an exam and 14 days before final. Last minute requests may get rescheduled for lack of room or for lack of enough proctors to administer exams.

3.  Instructor submits hard copies to ISS in E2-298 of exams for each student 48 hours prior to the exam time.

4.  Exam accommodations are provided to ensure equal educational access for qualified students with disabilities. Students have a right to accommodations in accordance with both Federal regulations and University policy. 

 

Arrangements to be made by the instructor include: 

1. Be available (or designate a TA) for questions during exams if typically provided to the rest of the class. 
    

2.  The DRC can provide test scribes or readers, as authorized for students with disability-related needs with advanced notice from you (at least one week prior to exams if possible). Please fill out the appropriate form found here.

3. Closed Caption Television (CCTV) enlarges hard copy documents, allowing students with visual impairments to access paper exams. Because the screen can be visible by others, the CCTV requires a separate room. The DRC will deliver and set up the CCTV at the exam location. Requests for a CCTV should be made at least 5 business days in advance if at all possible. We strongly encourage you to submit the DRC CCTV Request Form listing all of your class's exams early in the session. Please refer to the DRC student's accommodation letter for details about their individual accommodations. You can request the CCTV on this form.

 

**  Note: Test proctors are not required by law.  It is up to the instructor whether or not to designate one.

** If a test proctor IS NOT required, ISS will schedule a room and a time (information received from instructor via online form), and schedule student/s, and cc the instructor with room information via Google Calendar.  Instructor or their designate will meet the student(s) in the scheduled exam room and provide exam.  

** If the instructor requires a test proctor, ISS will schedule a room, time and test proctor and notify student/s and cc the instructor with testing information via Google Calendar.  The test proctor will meet the student/s in the scheduled exam room. Proctors are only provided for exams (not quizzes).
